54F with T2DM, HTN, hypothyroid presenting with 3 days of dysuria, frequency, no fever. No CVA tenderness. Urine dip positive for nitrites and leukocytes. No recent antibiotic use. Sulfa allergy documented — nitrofurantoin selected. Treated empirically for uncomplicated UTI. Urine culture sent. Instructed to return if symptoms worsen or fever develops.
